Your Appointment
First Visit
For your first visit with Dr Farjo please bring the following:
- Referral letter from your GP, family physician or other specialist if you are planning to get medicare rebate for the consultation;
- Medicare card, DVA card, and health fund details
- Ultrasound reports and films, blood test results and any other relevant information; and
- fill out and submit completed New Patient Registration Form before your appointment.
Referrals
Appointments must be covered by a valid referral if you are willing to claim a Medicare rebate.
A referral from your GP is valid for 12 months, a referral from a specialist is valid for 3 months.
Payment
Sometimes you may be required to undergo further investigations and/or provide you with
equipment or supplies to assist in your treatment, which may incur additional costs.
Fees are payable at time of consultation at which time we can also submit your Medicare rebate.
Confirming an Appointment
Few days prior to your appointment, reception will confirm your consultation time, and date with an SMS message. We ask that you promptly reply to the SMS to confirm your appointment. If you do not have a mobile phone, then we will contact you via the contact details you provided.
Waiting List
If there are no appointments available at a time that suits you, reception will offer to place you on the “Wait List”. If we have a cancellation, reception will contact you as soon as possible
Cancelling and changing appointments
If you need to cancel or change an appointment, we require at least 24 hours notice (business hours). This gives us time to contact the people on the “Wait List”.
